Most people arrive looking for a product when the real question is which category applies to them. Getting that wrong is more expensive than choosing the wrong plan within the right category, because it can leave you with coverage that does not do what you assumed it did.
Major medical coverage — an ACA Marketplace plan, an employer plan, Medicare, or Medicaid — is what protects you against the cost of serious illness or injury. Everything else on this page supplements it. A supplemental, indemnity, dental, vision, or short-term product is not a substitute for major medical coverage, and buying one believing otherwise is the most consequential mistake in this space.
The order of operations is: settle your major medical path first, then decide whether a supplemental product closes a gap you have actually identified in it.
A short set of questions usually resolves it. Are you 65 or approaching it, or on disability? Then Medicare timing rules apply and take priority, because their deadlines carry permanent penalties. Does anyone in your household have an offer of employer coverage? That affects Marketplace subsidy eligibility and should be evaluated before assuming financial help is available.
Is your household income low relative to your state's thresholds? Medicaid and CHIP have no enrollment window and should be checked first, since they are usually the better outcome where eligibility exists. Otherwise, an individual ACA Marketplace plan is generally the path, with premium tax credits determined by household income and county.
Only after that is settled do the supplemental questions make sense: whether a high deductible warrants an indemnity policy, whether dental and vision are worth separate coverage, whether a life insurance obligation is uncovered.
A coverage review starts with the information that determines the answer: household size and tax filing status, estimated income for the coverage year, the doctors and hospitals you want to keep, your prescriptions with dosages, and any employer coverage available to your household.
From there it is comparison work — options from represented carriers checked against your actual providers, medications, and expected use of care across a full year rather than a monthly premium.
Availability depends on your state and county, on eligibility determined by the Marketplace or the relevant agency, and on the carriers represented. A broker's comparison covers the carriers they are appointed with, which is not the same as every plan in your county; the official Marketplace preview shows the complete on-exchange menu.
Most coverage mistakes fall into a small number of recurring patterns, and nearly all of them are avoidable by knowing they exist.
The first is buying a supplemental product believing it is major medical coverage. An indemnity, accident, or short-term policy pays a defined benefit against defined triggers; it does not protect you against the open-ended cost of a serious illness. This is the most consequential error in the category.
The second is choosing by premium alone. A plan with the lowest monthly cost and a high deductible frequently costs more across a year with real medical use, and the households most likely to make this choice are often the ones least able to absorb the deductible.
The third is skipping verification. Enrolling without checking that your doctors are in that specific plan's network, and that your medications sit on its formulary at a workable tier, is how people discover in February that their plan does not work for them — at which point they generally cannot switch.
The fourth is missing a window. Special Enrollment Periods run about 60 days, Medicare's Initial Enrollment Period is seven months, and the Medigap open enrollment window is six months and does not return. Missed deadlines cause more expensive outcomes than wrong plan choices do.
The fifth is assuming ineligibility. A substantial number of people who believe they cannot afford coverage have never run an eligibility check and would qualify for meaningful financial help.
Not every product described here is available to every household, and the constraints come from three separate directions.
The first is licensing. Insurance is regulated state by state, and a producer can only advise and enrol in states where they hold an active license. If you are outside those states, that is a hard limit rather than a matter of preference.
The second is eligibility, which is determined by the Marketplace, Medicare, or a state agency rather than by any agent. Whether you qualify for premium tax credits, cost-sharing reductions, Medicaid, or CHIP depends on household income, tax household composition, immigration status, and whether anyone has an employer coverage offer that meets federal tests.
The third is carrier appointments. A broker can present products from the carriers they are appointed with, which is not the complete market. For ACA coverage specifically, the official Marketplace preview tool shows the full on-exchange menu for your ZIP code, and it is worth viewing alongside any comparison you are given.
Where a product is not available to you, the useful answer is which alternative applies rather than which product comes closest.
